This form is to be completed by individuals seeking media accreditation at an AFF tournament.

As part of the application process, and prior to signing this form, applicants are required to confirm that they have read and understood the following AFF documents:

• AFF National Integrity Framework
• AFF Code of Conduct
• Conditions of Entry

These documents are available on the Integrity and Policies section of the AFF website.

In addition, when collecting your accreditation at the venue, you must present:

• A valid Working With Children Check, and
• Evidence of completion of the Sport Integrity Australia Safeguarding Children and Young People in Sport Induction.
